Position Description

Landmark Health is a high growth, entrepreneurial, home-based provider team that is transforming healthcare by serving the extensive needs of the most chronically ill in our communities. The role of the urgentivist extender supervisor will be to lead a team of urgentivist extenders and to serve on the frontlines and as market lead for ALERT care delivering urgent medical care in the homes of complex, frail, and vulnerable patients. This position will assist with urgent visits under the traditional Landmark Health model and under the Alert Care model if available..

Through the integration of technology, a highly innovative care delivery model, and multidisciplinary clinical teams, Landmark delivers health management capabilities at scale for the frailest and most clinically complex segments of the population – the 3-5% of members that account for 20-30% of all national healthcare expenses annually.

As an urgentivist extender supervisor, your duties will be to:

  • Function as a day-to-day clinical leader by providing clinical decision support, guidance, and scheduling oversight including geographic deployment decisions to urgentivist extenders in managing urgent visits.
  • Provide oversight and to ensure best visit practices to avoid unnecessary ED transfers and hospital admissions. Assisting with determining appropriate cadence for the follow-up visits post urgent visit.
  • Perform needed urgent visits as needed to either provide support to the urgentivist extenders
  • Leverage the support of nurse care manager, behavioral health, social work, pharmacy, and dietician to meet patients’ medical, biopsychosocial, and financial needs
  • Coordinate with the market physicians and ideally across the continuum of care, including PCPs, ER physicians, hospitalists, and SNF providers in order to ensure smooth transitions and prevent readmissions
  • Perform regular skills training and provide specific education to the urgentivist extenders
  • Perform some administrative duties including monitoring and maintenance of urgentivist extender schedules and timecards. Other duties would include performing ride-a-longs and providing direct feedback to the team. Local idt liason- determining which idt to attend or how to have them present at a portion of each teams idt, provide education for any relevant LMW.
  • Meet at least monthly with the Landmark First medical directors, in addition to regular meetings with the local market RMD.
  • Completion of Nurse Practitioner (NP) or Physician Assistant (PA-C) accredited program
  • 5+ years of clinical NP or PA experience preferred
  • Prior clinical background for acute care and urgent care management
  • Some administrative or clinical oversight experience preferred
  • Knowledge of clinical standards of care
  • Current unrestricted NP or PA state license, and current state driver’s license
  • Access to reliable transportation to conduct patient visits required; if you are driving a vehicle, you must comply with all the terms of the Landmark Motor Vehicle Safety policy.
  • Passion for delivering care to patients with complex, chronic illnesses
  • Willingness to conduct house calls
