Nurse Practitioner

Nurse Practitioner/Physician Assistant, Long Term Care - Medford, OR

$30,000 Sign-on Bonus or $40,000 Student Loan Repayment

Role Overview:

• Provide primary care delivery and care coordination at home, nursing homes, assisted living, and senior housing
• Deliver cost-effective, quality care to assigned members
• Manage medical and behavioral conditions in collaboration with physicians
• Perform comprehensive assessments and document findings
• Conduct medication reviews and reconciliations
• Coordinate care across different levels and settings

Qualifications:

• Certified Nurse Practitioner or Physician Assistant
• Graduate of accredited programs with board certification
• Active and unrestricted state license
• Current DEA licensure/prescriptive authority
• Reliable transportation
• Ability to lift 30-pound bag and navigate various dwelling conditions

Preferred Qualifications:

• 1+ years of post-grad experience in Long Term Care
• Understanding of geriatrics, chronic illness, and advanced illness
• Proficient computer skills for medical documentation
• Ability to develop positive customer relationships
• Adaptability to change
