Position Description

Opportunity Overview:

Working in the town of Eugene, OR
15 -20 patients per day
This is a part-time position, one day a week
Nurse practitioners (NP) candidates and physician assistants (PA) are welcome to apply
Compensation and Benefits: Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated income range of $41,000 to $49,000 annually with no cap on productivity bonus income potential.

Responsibilities:

Drive high-quality, safe, and cost-effective patient care in post-acute care communities
Inpatient rounding in assisted living facility (ALF)
Collaborate with other clinician interdisciplinary teams and follow CMS guidelines
Work closely with hospice, palliative care, and home care agencies for safe patient transitions
Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines

Qualifications:

Board Certified FNP, AGACNP, ACNP, AGPCNP, or PA-C
Willing to train experienced candidates as well
Licensed in state
